Intake Procedure for Admission into Treatment Programs  
 
 
 
 
 
  • Pre-admission information generally is gathered over the telephone.
    • This takes approximately 20 minutes at which time we assess the appropriateness of admission to our program and assign a therapist that best fits your needs based on the information you provide.
    • Please advise the receptionist if you are requesting a direct referral to a specific therapist.
    • If it is determined you are not appropriate for our program, a referral will be made immediately.
  • Fees are discussed at this time.
    • Inform receptionist if you health insurance you wish to use. You will need to provide:
    • Name of Insurance Company or Third Party Payer
    • Subscriber Name
    • Subscriber Date of Birth
    • Contract Number
    • Group Number
    • Telephone Number on back of your card
    • Self Pay Rates
    • Sliding Scale Rates based on household income
  • Intake sessions require a two (2) hour appointment that includes time for completing forms prior to seeing your therapist.
  • A pre-admission packet is mailed to you within 24 hours, if time allows, that contains your Intake Appointment date and time. You will also receive a company brochure, AA Meeting List, Map to our facility and a Fee Schedule.
  • If you find it necessary to reschedule your appointment, we appreciate a 24 hour notice.

Procedure for Assessment Appointment
  • Assessment appointments (except D.O.T. Assessments), are one (1) hour in length.  
  • D.O.T. Assessments are two (2) hours in length. 
  • Plan to arrive 30 minutes prior to your appointment time to allow for completing forms.
 
Documents to bring to your first session 
  • Photographic identification: Driver License, State Identification Card, Passport, School Badge, etceteras.
  • Referral source name, address, telephone and fax number.
  • Documentation your referral source requires you to have us sign.
  • Payment for session. We accept cash, check, money order, major credit/debit cards.
